Histamine is a compound your body makes and also takes in from food. Most people clear dietary histamine without noticing. If you clear it slowly, it accumulates — and once it crosses your personal threshold, you get symptoms.
That threshold idea explains the thing that makes this so confusing: reactions depend on the total load, not on one culprit food. A glass of wine alone might be fine. Wine with aged cheese after a stressful week might not be. Nothing was "the" trigger; the bucket simply overflowed.
If you change only one thing, change how fresh your food is — not which foods you eat.
Despite the name, it is not an allergy. An allergy is your immune system misidentifying a specific protein as a threat and mounting an attack — which is why allergy testing can find it. Histamine intolerance is a clearance problem: the histamine is not being mistaken for anything, your body just cannot process it fast enough.
This is why people often arrive here after a clean allergy panel. The test wasn't wrong and the symptoms aren't imagined — the test was simply looking for a different mechanism.
The enzyme mostly responsible for breaking down histamine in the gut is diamine oxidase, usually shortened to DAO. When DAO activity is low, histamine from food passes into the bloodstream instead of being dismantled in the digestive tract. DAO can be reduced by gut inflammation, certain medications, genetics, or by being kept busy with other compounds — more on that below.
Because histamine receptors sit throughout the body — gut, skin, blood vessels, airways, brain — symptoms are rarely confined to digestion. That is often the clue that separates this from a purely digestive issue.
Histamine is produced by bacteria acting on protein. That process starts the moment food is harvested or slaughtered and continues, more slowly, in your refrigerator. Cooking does not reverse it — heat kills the bacteria but leaves the histamine behind.
The practical consequence is that the same food can sit in two different tiers depending on its age. Fresh-caught fish is one of the better proteins available to you. That identical fish, three days later, is one of the worst. No food list can capture this, which is why freshness is a habit rather than a lookup.

The inconsistency is characteristic. Because tolerance is cumulative and shifts with stress, sleep, hormones, and medications, a food can be fine on Monday and a problem on Friday. Many people conclude they must be imagining it. They usually are not — they are watching a threshold move.
A food can cause trouble through more than one route, which is why some things appear worse than their histamine content alone would suggest. Alcohol manages three of the four at once.
The direct route. Histamine forms during fermentation, aging, ripening, and spoilage — so the more time and bacteria involved, the higher the level.
Aged and blue cheeses · cured meats and sausages · fish that isn't freshly caught · sauerkraut and other fermented vegetables · vinegar and pickles · soy sauce · yeast extract · alcohol. Also a handful of vegetables that are naturally high even when fresh: tomatoes, spinach, eggplant, avocado.
Histamine has chemical relatives — putrescine, cadaverine, tyramine — that are broken down by the same DAO enzyme. DAO deals with them first. While it is occupied, your histamine sits in the queue. These foods aren't high in histamine themselves; they monopolize your capacity to clear it.
Legumes (lentils, beans, soy and soy products) · banana · pear · pineapple · kiwi · orange · grapefruit · papaya · raspberries · peanuts · wheat germ.
These prompt your own mast cells to release the histamine they have stored. The histamine involved is yours, not the food's — which is why DAO supplements don't help against this route.
Alcohol · strawberries · citrus fruits · tomatoes · chocolate and cocoa · walnuts and cashews · shellfish and crustaceans.
Substances that directly suppress the enzyme, lowering your ceiling for everything else you eat that day. Several common medications do this too — worth raising with your doctor or pharmacist rather than stopping anything yourself.
Alcohol and its breakdown product acetaldehyde · theobromine (cocoa, chocolate, energy drinks) · mate tea · various medications.
A fifth factor worth knowing: hot spices — chili, pepper, curry — increase gut permeability, letting more histamine cross into the bloodstream. Alcohol does this as well.
A low-histamine diet is a diagnostic tool, not a destination. The goal is not lifelong restriction — it is to quiet things down long enough to see whether histamine is involved, then rebuild the widest diet you can tolerate.
Phase 1 — Elimination (2 to 4 weeks)
Cut the "avoid" foods and tighten up freshness at the same time. Both matter; doing only one muddies the result. Keep a simple log of what you ate and how you felt — patterns here are easy to miss from memory. If nothing has improved after four weeks, extending further is rarely useful. That is information too.
Phase 2 — Reintroduction (the important part)
Add back one food at a time, in a small portion, on a day when the rest of what you eat is low-histamine. Wait 24 to 48 hours before judging it — reactions are often delayed. If it goes well, try a larger portion before declaring it safe, since tolerance is dose-dependent.
Skipping this phase is the most common mistake. People feel better, stay frightened of the whole list, and end up eating fifteen foods for years. Most people tolerate considerably more than the strict list allows.
Phase 3 — Your own list
The endpoint is a personal map that will not match anyone else's, and will keep shifting. Tolerance tends to be better when you are well-rested and worse during illness, stress, or hormonal shifts. Think of it as a budget you spend across a day rather than a list of banned items.
If a careful low-histamine trial doesn't produce lasting relief, the answer is usually that you are on the wrong framework — not that you did it badly.
Mast cell activation. The source guidance for this page is explicit that its list is not sufficient for mast cell activation disorders, which are frequently mistaken for histamine intolerance. In these conditions mast cells release histamine and other mediators too readily, so the problem isn't only dietary intake and DAO supplements don't help much. Symptoms are typically broader and more reactive, sometimes triggered by heat, exercise, or scent rather than food. This needs proper medical assessment.
Carbohydrate fermentation. If your symptoms are mainly bloating, gas, and pain — without the flushing, headaches, or sleep disruption — FODMAPs are the more likely explanation.
Worth knowing that the two lists conflict in places. Standard low-FODMAP advice recommends aged cheeses, spinach, and batch-cooked leftovers, all of which are high-histamine. If you are following one and feel partially better, it may be worth cross-referencing the other.
